Welcome to Access FHM-Cardiology
Preventive medicine has always been central to care at Foxhall Medicine. This approach focuses on maintaining health through lifestyle guidance, immunizations, laboratory testing, risk management, and regular follow-up. Developing personalized preventive care plans requires additional physician time and dedicated staff support.
Beginning January 1, 2026, all patients of Dr. Joshua S. Yamamoto are required to pay the Access FHM Cardiology fee to remain patients of his practice. This fee covers the additional staffing required to maintain the same level of care our patients have come to expect.
Enrollment Fees and Information
Access FHM Cardiology is an annual membership program that automatically renews on the date of enrollment.
Established Patients
- $325.00 per patient annually, paid in full at enrollment
- $360.00 per patient annually, paid monthly at $30.00.
If you are experiencing financial difficulty and need to discuss alternatives, don’t hesitate to contact our practice administrator, Angela Jones, at ajones@foxhallmedicine.com. All calls are confidential and not included in your patient chart.
New Patients
- $650.00 Initial Visit Fee for New Patient Intake-Must be paid before scheduling your first appointment.
Decline Enrollment
Patients who prefer not to pay the access fee will have 90 days to transfer care to another cardiologist. Dr. Yamamoto will continue to provide care for any emergency issues during those 90 days and will provide a 90-day refill for all medications. As required by law, FHM will provide a copy of your medical records upon written request
