This annual membership program automatically renews each year on the date of registration. We do not charge an additional fee for those patients who see Dr. Thomas and Dr. Yamamoto.

Individual Patient Pricing:

• $1,000.00-Annual fee, paid in full upon enrollment and automatically renewed each year.
• $1,200.00-Annual fee, paid in automatic monthly payments of $100.00 and automatically renewed each year.

Couples/Additional Family Member Pricing:

For those couples and families who are patients of the practice, we offer the following options:

• $500.00- additional annual fee for spouse/partner/child, paid in full upon enrollment and automatically renewed yearly.
• $ 600.00- additional annual fee for spouse/partner/child, paid in automatic monthly payments of $50.00 and automatically renewed yearly.
